I've been on my period for 45 days. Heavy bleeding, blood clots like crazy. I've had this problem before.?

Last time, I was on my period for 7 months. YES, I said months. It's true. I ended up getting a blood transfusion, and finding out I had gobs of cysts in my ovaries. Should I go to the ER? I'm VERY light headed. My heart races when I stand up and walk. My vision goes dark where I can barely see. I have no money. I have no insurance. I just don't want to go to the hospital without having something REALLY wrong with me and end up paying a bill that I didn't need. I already have 2 huge hospital bills from the previous 2 months. I don't know what to do.
